
Breast Reduction: Procedure, Recovery, and Medical Benefits
A fuller bust may be the dream of many women, but there are also quite a few who seek exactly the opposite result, choosing breast reduction as the only solution for excessively large (oversized) and sagging breasts—even at a young age.
What is breast reduction and in which cases is it performed?
It is a procedure sought by women of all ages who have large and heavy breasts (macromastia – gigantomastia). The goal of breast reduction is not only to decrease the size of the breasts, but also to improve their shape and projection, as well as to correct their position, so that the new breasts are more symmetrical, proportionate to the rest of the female body, more attractive, and more youthful.
The excessive weight of large breasts can cause chronic pain in the neck, back, and lower back, as well as breast sagging, limitations in physical activities, and difficulties in daily life. As a result, the decision to undergo breast reduction is driven by a combination of aesthetic, functional, and emotional concerns, making it a deeply personal choice.
At what age can breast reduction be performed? Can it be done during adolescence?
There is no ideal age for the procedure. An adolescent girl can be considered for breast reduction, with parental consent, but the basic principles that guide my philosophy as a plastic surgeon are as follows:
- Full breast development
Adolescents need to wait until their breasts have fully developed to avoid the possibility of reversing the aesthetic result. A necessary condition for performing the procedure in teenagers is that there have been no significant changes in breast size for at least one year and that at least two years have passed since the first menstruation.
- Emotional maturity
Body image during adolescence is a crucial aspect of self-identity, strongly linked to self-esteem, and can change easily. Therefore, minor candidates for breast reduction need to be emotionally mature, have realistic expectations, be fully informed about the procedure, be able to follow the plastic surgeon’s instructions, and adopt a healthy lifestyle postoperatively.
- Future plans
It is important for the plastic surgeon to be aware of any plans for significant weight loss, to ensure that the patient will be satisfied with the result both immediately after surgery and in the future.
Before Breast Reduction: The Importance of the Preoperative Consultation
During the preoperative consultation, the plastic surgeon will evaluate factors such as breast size, shape, and position, the size and position of the areola, skin quality, and body measurements (height, weight, chest width, etc.).
Based on these factors, the surgeon will determine the ideal technique—or combination of techniques—for the patient and will create a personalized surgical plan to ensure the procedure is tailored to the individual needs of her body.
Additionally, the plastic surgeon will take a detailed medical history, focusing on your health status, any medications or dietary supplements you may be taking, and your lifestyle (including daily habits and type of work), in order to design the postoperative plan and inform you about the recovery process.
As part of proper preparation for the surgery, on the day before the procedure, you will visit the clinic for photographs and careful preoperative planning. Any questions you have will be answered so that you enter the surgery with calmness, confidence, and peace of mind, while participating in shaping the final outcome together with the plastic surgeon.
On the morning of the surgery, upon your arrival at the clinic, you will undergo the necessary preoperative checks to ensure safety and a faster recovery. You will then be transferred, wearing special surgical attire, to the operating room, where the anesthesiologist will be waiting for you.
The Surgical Procedure: How is Breast Reduction Performed?
There are various breast reduction techniques, but the most modern approach I use is the vertical breast reduction (vertical mastopexy), which involves only two incisions: one around the nipple–areola complex (donut incision / periareolar incision) and a vertical incision (lollipop incision). This technique minimizes scarring, avoiding the large horizontal and unsightly anchor-type incisions (anchor incision) of the past—which often deterred women from choosing breast reduction—and ensures a better aesthetic result with precision and high predictability.
During the procedure, targeted amounts of skin, fat, and part of the mammary gland are removed, and the breasts are reshaped. The nipple and areola remain attached to the remaining breast tissue to preserve blood supply and nerve function, ensuring sensitivity and functionality, and are repositioned to the ideal height. In extremely rare and complex cases of gigantomastia, the nipple may need to be transferred as a free nipple graft.
Because large breasts are usually sagging, breast reduction often includes a breast lift to achieve a completely natural outcome—giving the breasts a teardrop shape—and to maximize long-term results.
In a well-executed procedure, the right and left breasts are symmetrical in size and shape at the end of surgery. For this reason, all stages of the operation emphasize achieving breast symmetry through careful measurements.
The surgery is performed under general anesthesia and lasts approximately 2 to 4 hours, depending on the extent of the procedure. It is always carried out in a fully equipped medical facility that meets safety standards and high-quality healthcare service requirements.
When is Breast Reduction Combined with Liposuction?
Often, during the procedure, liposuction is performed on the sides of the breasts, because large breasts frequently have a fatty extension toward the armpit, known as the “axillary tail.” Combining breast reduction with liposuction ensures an improved breast contour and better harmony with the female chest, providing an excellent aesthetic result. Much more rarely, this issue is addressed through direct excision.
To what extent can breast reduction be performed?
The guiding principle for the plastic surgeon during breast reshaping is to achieve symmetry between the breasts and proportion with the rest of the female body, as well as proper healing of the skin and nipples. While the patient’s wishes are taken into account, it is generally not recommended to reduce the breasts by more than three cup sizes.
What is the recovery like after breast reduction?
After the surgery, the patient is fitted with a special elastic sports-type bra, which she will need to wear for approximately one month. This provides breast support, promotes faster healing, and helps achieve the ideal aesthetic result. Usually, an overnight stay in the clinic is not required, so discharge is given on the same day.
Most women undergoing breast reduction are pleasantly surprised to find that post-operative pain is much less than expected—manageable with mild painkillers—or sometimes nonexistent. It is, however, crucial that the first few postoperative days are dedicated to breast care, following the instructions provided by the plastic surgeon, and to rest, to ensure smooth healing. Any postoperative swelling (edema)—which tends to be greater in cases requiring more extensive surgical intervention due to the size of the breasts—begins to subside within the following days.
Return to light office work is possible a few days after surgery. For physically demanding work, the plastic surgeon will inform the patient preoperatively about how long she should refrain from such activities.
Raising the arms above shoulder level, going to the gym, lifting weights, and any intense physical activity should be avoided during the early postoperative period. Approximately one month after breast reduction, the incisions will have healed, and the patient’s range of motion will have returned, allowing a full return to normal activity.
Breast Reduction and Breastfeeding: Is Nursing Possible After the Procedure?
Modern breast reduction fully preserves the ability to breastfeed in case of pregnancy after the procedure. Studies have shown that breastfeeding success rates are similar between women who have undergone a successful breast reduction and those who have not had the surgery.
In every breast reduction procedure, I take care to protect the milk ducts, nerves, and breast tissue as much as possible so that both mother and baby can benefit from breastfeeding. However, there is a possibility of reduced milk supply, which depends on the technique used and individual healing.
When Will I See the Results of Breast Reduction?
The patient will notice a change in her body immediately after the surgery, on the operating table, especially if a significant reduction in size was performed. However, the aesthetic result of breast reduction becomes more apparent after a few weeks, once swelling subsides, and with proper postoperative care, there is significant potential for further optimization.
Long-term benefits of the procedure include a more youthful appearance, a more active lifestyle, enhanced self-confidence, and improved overall health.
How Does Breast Reduction Improve a Woman’s Health?
It is no coincidence that breast reduction ranks among the procedures with the highest levels of postoperative satisfaction. Beyond its transformative effect on the female body, it provides significant health benefits. The patient:

Will Breast Reduction Help Me Lose Weight?
Breast reduction should not be considered a substitute for weight loss. However, there are two relevant points. First, in cases of gigantomastia, the weight removed from each breast can reach one kilogram or more. Second, having smaller, more manageable breasts increases comfort and mobility, allowing the patient to incorporate physical activity into her daily routine—an adjustment that, combined with a healthy diet, can contribute to weight loss.
Are the Results of Breast Reduction Permanent? Can My Breasts Grow Again?
Breast reduction provides a permanent aesthetic result. However, a woman’s breasts may slightly increase in size again due to pregnancy, significant weight gain, hormonal changes, or the effects of gravity. Unfortunately, these factors—similar to what can affect a breast lift—can influence breast size, but by maintaining a healthy lifestyle, the likelihood of the breasts returning to their pre-surgery size is extremely low.
What Is the Cost of Breast Reduction?
A common concern for women seeking relief from excessively large breasts is the question of “breast reduction cost.”
The factors that determine the final price of breast reduction include:
- The technique used, which depends on breast size and the patient’s anatomical characteristics
- The extent and complexity of the problem
- The cost of the surgery itself (surgeon and anesthesiologist fees, clinic costs, surgical supplies)
- The duration of hospitalization
- Combination with other procedures (e.g., liposuction of the sides of the breasts or another body contouring surgery, such as a tummy tuck) for optimal aesthetic results
During the first preoperative consultation, I take the time to provide patients with a detailed overview of the total cost of the procedure.
In cases of gigantomastia, which causes chronic problems, part of the cost may be covered by insurance, as it is considered a medically necessary reconstructive procedure. You should consult your chosen plastic surgeon regarding the requirements and process, which involves collecting documentation (medical reports, photographs, etc.) and submitting a request to your insurance provider.
In any case, when deciding on breast reduction, it is important to prioritize the surgeon’s scientific training, expertise, and experience, as well as the quality of healthcare provided, over the cost alone.
